Registration of New Patients

“We welcome requests for registration from patients already living in or practice area, or who are moving in to it. You can register by present your medical card or completing a form GMS1 which is available from the reception desk. Prospective patients can also register online by clicking the link secure pre-registration.

Patients are registered with the practice, rather than an individual doctor and may not always see the same doctor. However, patients can express a preference of practitioner. We will try to comply with your request, but it might not always be possible, in which case we will explain the reasons. If you move out of our area, we regret that you must register with another practice.

In addition:

  • registered patients aged 16-75 who have not been seen for 3 years may request a consultation
  • registered patients aged over 75 years who have not been seen in the previous 12 months may request a consultation
  • if you are unable to attend the surgery for these checks because of your medical condition a home visit may be arranged

Prospective patients can send registration information such as name, address and previous GP details to Whitchurch Village Practice before they attend the practice to register, this can save both patient and Practice time and ensures all relevant information is collected.
